NO PURCHASE NECESSARY TO ENTER OR WIN. THIS SWEEPSTAKES IS OPEN ONLY TO LEGAL RESIDENTS OF THE 48 CONTIGUOUS UNITED STATES AND THE DISTRICT OF COLUMBIA WHO ARE 14 YEARS OF AGE OR OLDER. SWEEPSTAKES STARTS ON OCTOBER 14, 2004 12:00 A.M. EST. AND ENDS ON DECEMBER 31, 2004 11:59 EST.
There are two ways to enter. To enter on-line, fill out the official on-line entry form above including your full name, address, city, state, zip code, daytime phone number, e-mail address and birth date. To enter by mail, hand print your full name, address, city, state, zip code, daytime phone number, e-mail address (if applicable), jacket size and pants size on a 3x5 piece of paper and mail to: SUM 41 / EPIPHONE GUITAR Sweepstakes c/o The Island Def Jam Music Group, 825 8th Avenue, New York, New York 10019 Attention: SUM 41 / EPIPHONE GUITAR Sweepstakes Entries / New Media Department. On-line entries must be received between OCTOBER 14, 2004 12:00 A.M. EST. AND ON DECEMBER 31, 2004. Each mail-in entry must be stamped and mailed separately, postmarked by December 31, 2004 and received by January 5, 2005. Limit one entry per person, regardless of entry method. Multiple entries, or entries generated by script, macro, bot, or other automated means will be disqualified. If you are under the age of 18, you must obtain your parent or legal guardian's consent to enter this sweepstakes. Any entry information collected from the sweepstakes shall be used only in a manner consistent with the consent given by entrants at the time of entry and with The Island Def Jam Music Group's On-Line Privacy Policy, which can be found at:

You must be a legal resident of the contiguous 48 United States and the District of Columbia and be at least 14 years old or older as of your date of entry. Employees and agents of The Universal Music Group, The Island Def Jam Music Group, Levis and each of their respective affiliate, subsidiary, and parent companies, and advertising and promotions agencies and their immediate families (spouses, parents, siblings, children and their spouses) are ineligible to enter or win.
Prize. One Grand Prize Winner will receive: one Epiphone Les Paul Jr. Guitar, autographed by Sum 41. Approximate retail value for all of the above items, in the aggregate: $300. No transfer, substitution or cash alternative permitted, except by Sponsor in its sole discretion. If the winning entry was made on-line, the entry will be deemed submitted by the e-mail account holder associated with such entry, as solely determined by Sponsor. Any taxes or any fees or costs associated with the Grand Prize are the sole responsibility of each Grand Prize Winners. Each Grand Prize Winner (or his or her parent or legal guardian) will be required to complete and return an IRS W-9 form. Prizes are awarded “as is†with no warranty or guarantee, either express or implied.
Each potential Grand Prize Winner will be selected in a random drawing from all eligible entries received, on or about January 5, 2005 by Sponsor or its designee, whose decisions will be final. Each potential Grand Prize Winner will be notified by e-mail and/or by phone on or about January 5, 2005. If Sponsor or its designee is unable to contact any potential Grand Prize Winner by e-mail or phone (using the contact information provided at the time of entry) and/or if any potential Grand Prize Winner fails to respond to notification within two (2) business days of issuance, the potential winner(s) will be disqualified and forfeit his or her right to a prize, and an alternate winner will be selected. Within ten (10) days of the date of the notification letter, each potential Grand Prize Winner (or his or her parent or legal guardian if he or she is a minor) will be required to execute (and have his or her signature notarized) an affidavit of eligibility, general release of liability and consent (in writing, if requested) to the use of his or her name, voice, likeness, and/or photograph, and biographical information for promotional purposes, including on-line announcements, without additional compensation of any kind, except where prohibited by law. If any Grand Prize Winner is a minor in his or her state of legal residence, such Grand Prize will be awarded in the name of the Grand Prize Winner's parent or legal guardian. Odds of winning depend upon the number of eligible entries received by Sponsor.
General Conditions. Sponsor, The Universal Music Group, Epiphone, any Internet access providers, each of their respective parent companies, affiliates, subsidiaries, advertising and promotion agencies, and all of their agents, officers, directors, shareholders, employees and agents (collectively, “Released Partiesâ€) disclaim any responsibility for incorrect or inaccurate entry of entry information, human error, technical malfunctions, lost / delayed data transmission, omission, interruption, deletion, defect, line failures of any telephone network, computer equipment, software or any combination thereof, inability to access any web site or download materials, damage to a user's computer system (hardware and software) due to participation in this sweepstakes, or late, lost, incomplete, misdirected, mechanically reproduced, illegible or postage due entries or mail. If in the opinion of Sponsor, there is any suspected or actual evidence of electronic or non-electronic tampering with any portion of this sweepstakes, or if technical difficulties compromise the integrity of this sweepstakes, Sponsor reserves the right to terminate this sweepstakes, and award the prizes from among all eligible, non-suspect entries received as of the action requiring such termination. Any attempt to deliberately damage any web site or undermine the legitimate operation of the sweepstakes is unlawful and subject to legal action by the Sponsor or its designees.
Entrants agree to hold harmless the Released Parties from and against all claims and damages arising in connection with their participation and/or entry in this sweepstakes and/or the awarding or use of any prizes awarded.
Winner's List. Requests received after January 5, 2004 will not be honored. This sweepstakes shall be governed by the laws of the State of New York of the United States of America and is subject to all applicable federal, state and local laws. Void where prohibited by law.
